Join us for a special event in recognition of National Healthcare Decisions Day (NHDD) , while we learn about navigating your health care journey, five key documents for advanced care planning, ACP, POLST, and next steps.

✔️ Resource tables with local service providers, including medical, legal, and caregiving support

Participants will be able to identify their end-of-life values and goals of care and acquire the communication skills necessary to make informed healthcare planning decisions!
